Some are new, some are combinations of old and new, and some have been around for years.  The most important thing to keep in mind is to maintain focus. There must be a plan that is aggressively maintained by good managers lead by outstanding leaders.  The ideas must focus on the customer... remember "nothing happens without customers."  The tools must be used in a manner that will produce results, not just pretty graphs.  If graphs and statistics are part of the process they need to be focused on achieving a business result, not to show how good you are doing.
